All required forms to take part in the La Pietra Athletic Program can be downloaded below and will need to be uploaded to Magnus Health, found in the Resources tab of your parent Blackbaud account. For more information on each of the required forms, click here.

To participate on any sports team, download and complete these Physical Exam and Athletic Forms and upload them to Magnus Health, found in the Resources tab of your parent Blackbaud account.
