The Winter Sports Sign-up form is a Google Form that lists all of our La Pietra and Pac-5 sports offerings for the winter sports season. Students who would like to participate are requested to complete the form so that the athletic department and verify that all required forms and requirements for interscholastic athletic participation have been completed.

All required forms to take part in the La Pietra Lady Panthers Athletic Program can be downloaded below and will need to be uploaded to Magnus Health. For more information on each of the required forms, click here.

To participate on any sports team, download and complete these Athletic Student Health Forms and upload them to Magnus Health.
