Immunizations & Health Forms

Hawaii State Law requires all students to meet physical exam and immunization requirements before they may attend a public or private school in Hawaii. Students who do not complete the health requirements by the first day of school will not be allowed to attend school until they are met. Once forms are complete, submit them our main office prior to the first day of school.

Health Requirements
  1. Tuberculosis (TB) Clearance: A Tuberculin (Mantoux) test within the last 12 months before the first day of school.
  2. Physical Examination: A physical exam must be completed within 12 months before the first day of school.
  3. Immunizations: Complete series of immunizations based on age and interval requirements are required.

New Students. All new students to La Pietra must have a TB clearance, annual physical exam and updated immunizations completed prior to the first day of school. Schedule an appointment with your doctor and ask your daughter’s physician to complete the Student Health Record Form 14. If your daughter will participate in school sports, also ask your doctor to complete the Physical Examination for Athletes Form in addition to the Student Health Record Form 14.

Returning Students. All returning students must have a current health and immunization record in their student file. For student athletes, a physical exam must have been completed within the last 12 months prior to the first day of school. Simply ask your doctor to complete the Student Health Record Form 14 and the Physical Examination for Athletes Form (if participating in school sports).
