New Students

Complete Online Authorization Form

Complete the 2018-19 Online Authorization Form.

The form provides consent to: offer over the counter medication, display information in the online Student/Parent directory, release name and photo for publicity of La Pietra’s services, abide by and agree to the 2018-19 La Pietra Handbook, and attend any off-campus field trips.

Due: July 31, 2018

Complete Chromebook Agreement Form

Complete the 2018-19 Chromebook Agreement Form.

The agreement form details the expectations, responsibilities and care of the Chromebook issued to your daughter. Chromebooks will not be distributed if the form is not completed. Click here for more information.

For New Students, the Chromebooks will be distributed during the New Student Orientation on July 31, 2018.

If a student is not available on the given dates, Chromebooks will be distributed during the first week of school.

Due: At the time of Chromebook distribution or July 31, 2018 to Main Office

Review Locker Guidelines

Review the 2018-19 Locker Guidelines. Each student may select a locker within their designated grade location to keep their school books, supplies, and personal belongings during the school year. New students may select their locker on during the New Student Orientation on July 31.

Be sure to bring your lock with you on July 31, go to your designated locker area, select a locker, place your lock on the locker, and record your locker number and code to the Main Office.

If a student is unavailable to select their locker on July 31, they can select it during the first week of school.

Athletic lockers will be selected during the first week of your daughter’s term in P.E.

Complete Student Health Form

Complete the Student Record Health Form 14 and obtain a copy of latest immunization records. The Health Form is required by the Hawaii State Department of Health. Students must have their updated immunization completed within the last 12 months prior to the first day of school. Print the form and ask your doctor to complete at the time of the exam.

Upon completion, please scan and e-mail to info@lapietra.edu or mail to La Pietra's Main Office.

Complete TB Test

Complete TB (tuberculin) test and obtain results for all new students. Your doctor's office may administer this test and have the results recorded on the Student Health Record Form 14.

La Pietra requires a physical TB Skin Test. Returning students do not need to retake TB test.

Upon completion, please scan and e-mail to info@lapietra.edu or mail to La Pietra's Main Office.

Complete Physical Exam Form - Athletes Only

For ALL NEW STUDENT-ATHLETES in grades 6-12 who plan or will participate on a La Pietra, Pac-5, or CSAL (6th Grade League) team.

Complete the Emergency Information Section, print the Physical Exam Form, and ask your doctor to complete the top portion of this form at the time of her physical exam.

Upon completion, you may scan and e-mail to info@lapietra.edu or mail to La Pietra's main office.

Complete Concussion Forms - Athletes Only

For all new student-athletes in grades 7-12 who plan or will participate on a La Pietra or Pac-5 team.

  1. Complete and Sign Concussion Testing Consent Form
  2. Complete the Pre-Impact Concussion Worksheet
  3. Bring both forms with you to the concussion testing dates listed below or submit to the main office prior to your selected testing date.

Concussion Testing Schedule:

  • Thursday, July 19, 10:00 am - 2:00 pm, Conference Room
  • Friday, July 20, 10:00 am - 2:00 pm, Conference Room
  • Monday, August 6, 3:15 pm - 5:00 pm, Location: TBD
  • Tuesday, August 7, 3:15 pm - 5:00 pm, Location: TBD

All new student-athletes in grades 7 - 12 must complete testing prior to any practices, scrimmages, and games. You do not need to register to attend any of the dates listed above.

Due: At the time of your selected concussion testing date to the person administering the test OR to the main office prior to your selected testing date.

Complete Athletic Participation Form - Athletes Only

For all student-athletes in grades 6-12 who plan or will participate on a La Pietra, Pac-5, or CSAL team during the 2018-2019 School Year.

Complete the Athletic Participation Form. Upon completion, you may scan and e-mail to info@lapietra.edu or mail to La Pietra's main office.

Submit Tuition Payment

If applicable, submit tuition payment through My BackPack or to the main office. My BackPack is a website portal that La Pietra uses for profile information, class schedules, academic grades, and tuition statements. A separate e-mail with your login information to My BackPack should have been sent to you during the first week of July. If not, please contact the main office at 922-2744 or e-mail info@lapietra.edu.

Due: July 31, 2018 through My BackPack or to the main office.

Submit Parking Application - if applicable

Submit a 2018-2019 parking application with required documents and payment - if applicable.

Students planning on driving to school must submit a parking application with payment. Permits are issued in order received with priority given to seniors.

Upon completion, save/scan and e-mail application to info@lapietra.eduor mail to La Pietra’s Main Office.

Purchase Textbooks

Purchase Textbooks from La Pietra's MBS Direct Online Bookstore.

  1. Review your student schedule available in your My BackPack account (under the Academic Info heading) to determine the textbooks you will need. A separate e-mail with your login information to My BackPack should have been sent to you during the first week of July. If not, please contact the main office at 922-2744 or e-mail info@lapietra.edu.

  2. Using the TEXTBOOK LIST and your schedule, order your books through MBS DIRECT.

Only for Middle School Students. Students entering grades 6-8, should read at least one book from the list and be ready to participate in an activity during the first week of the new school year.

Learn About La Pietra's After School Strings Program

Whether a beginner or a seasoned player, you are invited to learn or improve your technique at La Pietra's After-School Strings Program! Two sessions will be offered, featuring two levels that meet on Tuesdays and Fridays every week.
